Digital Care App
Our online application enables care managers, legal representatives, care team’s and families have access to records, care plans and risk assessments 24/7. Our application hosts medication charts, accident and incident reporting, as well as task allocation, supervisions and appraisals. Each member of staff can access their rota, annual leave records and timesheets. Our online care application complies with CIW and GDPR legislation.

Bespoke Personal Care plans

Resolution Care Services has developed a personal care plan, formulated with the individuals and their representatives.

Progress is different for every person we support, therefore, goals are unique. We build on people’s strength not their disability.

The bespoke personal care plan offers a benchmark for both the individual and the care team supporting the person; to achieve outcome based, active support.

All Personal Care Plans are developed in line with the core principles of The Social Services & Wellbeing Act 2014.

Personal Outcome Wheel

Supporting Positive Behaviour

We believe that challenging behaviour should not be a barrier to experiencing a full and happy life. We work in partnerships to develop a behaviour support plan, ensuring the person’s communication needs are met. At Resolution Care Services we train all our staff in Positive Behavioural Support techniques and have extensive experience in helping people find new ways of dealing with challenging behaviour in a holistic and positive way. Adults may display behaviours which may put themselves or others at risk, or which may prevent the use of ordinary community facilities or a normal home life. This behaviour may be in the form of aggression, self-injury, stereotyped behaviour or disruptive and destructive behaviours, such as: hitting, kicking, spitting, shouting and destroying objects. At Resolution Care Services, our specialist and trained care teams will also provide support in the following areas, aiming to reduce repeated incidents of challenging behaviours and ensure the wellbeing and safety of all concerned.

  • Incident Recording
  • Incident Investigation
  • Post Incident de-brief
  • Post Incident Support
  • Incident Monitoring
