What Happens After a Referral?

Once we receive your referral, we will:

  • Assess the Individual's Needs and desired outcomes

    We will schedule an initial assessment, which can take place over the phone or in person, to understand the individual's specific needs, preferences, and circumstances.

  • Develop a Tailored Care Plan

    We will work with the individual and their family or support team to create a personalised care plan. This plan will reflect their goals and ensure all aspects of their physical, emotional, and social well-being are supported.

  • Assign a Dedicated Care Team

    Once the care plan is finaliSed, we will assign a dedicated, experienced care team to provide support that aligns with the individual’s unique needs, including any specialist care required for complex health conditions, mental health, or rehabilitation.
