At Resolution Care Services we provide fully managed, complex care support packages through the provision of expertly trained care staff, if required; overseen by our specialist clinical lead and social care managers.

Our clinical care team provide support and advice where required and in conjunction with the individuals own multidisciplinary team.

They will also be on-hand to advise and train support staff, where clinical input is required, including peg feeding, overseeing medication administration, providing personalised clinical care plans, DOLS and behavioural support plans.

Initial contact and meeting

We will then arrange an initial meeting at a convenient time to you, in order to find out more about your care or support needs. In this meeting we will aim to gather the correct information to see how we can provide the correct level of care & support. We listen to your wishes, feelings and views to understand your care and support needs and also assess any risks to ensure that support is delivered safely.

After the initial meeting takes place, we will aim to provide feedback and a proposal and decision within 7 days.

Once a decision has been agreed, an individual contract will be sent, outlining the terms and conditions and costings of the service to be provided.

Personal Care Plan and Risk Assessment 

Resolution Care Services has developed a personal plan, formulated with you and anyone else in your care. Your personal plan is specific to you and your needs and we will spend time ensuring the correct information.

Progress is different for every person we support therefore, goals are unique. We build on people’s strength not their disability. The personal plan offers a benchmark for both the individual and the care team supporting the person; to achieve outcome based, active support.
